Care Coordination makes it possible for many people who would otherwise need residential care to stay in their own homes or community.

We work in partnership with the client, families/whanau, carers and support network to provide client-focused care and solutions to optimise client independence, focusing on collaboration, elimination of duplication and community-based solutions.

Family support

Confidential counselling services are offered in both individual and group sessions.

We also provide information, reassurance, resources and strategies to help deal with grief and loss and, if needed, we can refer clients to other health services.

Our Family Support counselling service is free of charge to patients and their families, as is our Korowai programme which provides education for those caring for someone with a life-limiting illness.

Services of Remembrance are held during the year with families invited to attend nine to 12 months after the death of a loved one.

Appointments can be made between 8.30am and 5pm Monday to Friday, with some flexibility depending on need.
